Lab Results Authorization
Laboratory results are reported to the clinician who ordered the test. This request directs that clinician and the laboratory to release your results to you, through Octo Health, so they land in your record. It is presented in the app before your first lab order and a copy is kept in your account.
Effective: 1 September 2026
Where this applies
This document is for members in the United States, where lab panels are available. If you live elsewhere, the App shows you the version for your country before your first order.
1. What I am requesting
I request that the laboratory network Octo Health works with, the laboratories in it, and the licensed clinicians who order and review my tests (each an “Ordering Clinician”) release to me, through Octo Health as my authorized agent, all completed results from panels ordered in connection with my Octo Health membership so that my results are delivered into my Octo Health record and available to me through the Octo Health App.
2. What I Authorize
∙ The Ordering Clinician sees my results first. Results reach them before, or at the same time as, they reach me through Octo Health. Clinically significant values are handled under that clinician’s protocols, which may include contacting me directly.
∙ Receiving results is not medical advice. Delivering them to me through Octo Health is for my information and my records. It is not advice, diagnosis or treatment, and it does not replace talking to a healthcare professional.
∙ Questions about what a result means go to a healthcare professional: the Ordering Clinician or my own provider. Octo Health staff are not clinicians.
∙ Results can contain unexpected or distressing information. Opening them in the App is my choice and cannot be undone. If I would rather receive results only through a physician, I will not order lab panels in connection with Octo Health.
∙ Samples occasionally produce incomplete or inaccurate values. Results are produced by the laboratories and reviewed by the Ordering Clinician. Octo Health does not warrant their accuracy, completeness or timeliness.
∙ Exporting or sharing my results is my choice and my responsibility.
∙ Once the results are released to Octo Health, the results in the Octo Health App will not be protected by HIPAA. They will be protected by the Octo Health Terms of Service and Privacy Policy.
3. Changing my mind
This Authorization applies to lab orders during my membership until I revoke it, which I can do at any time, in the App or by emailing members@octohealth.com with REVOKE AUTHORIZATION in the subject line. Any clear written request to revoke will be given effect, whether or not it follows this format. Revocation is effective on receipt and is implemented within ten business days, works going forward, and does not undo results already delivered through Octo Health. I can request a copy of this Authorization at any time.
Questions about any of this go to legal@octohealth.com.
