Lab Testing Consent
This is presented with the Lab Results Authorization before your first lab order, and a copy is kept in your account. It is required in order to have panels performed, because the laboratory network and its clinicians cannot provide the service without this information. It does not affect your use of the rest of the app.
Effective: 1 September 2026
TELEHEALTH SERVICES ARE NOT INTENDED TO BE USED FOR URGENT OR EMERGENT HEALTH CARE NEEDS. IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, PLEASE DIAL 911.
TELEHEALTH SERVICES ARE PROVIDED BY THIRD PARTY LICENSED HEALTHCARE PROFESSIONALS WHO ARE AUTHORIZED TO PROVIDE SUCH SERVICES.
Where this applies
This document is for members in the United States, where lab panels are available. If you live elsewhere, the app shows you the version for your country before your first order.
1. What You agree to share
You authorize Octo Health to send the laboratory network it works with and its affiliated licensed clinicians (the “Lab Providers”) what the Lab Providers need to serve you: your name, date of birth, biological sex and contact details; the intake information you give in the lab ordering flow, including relevant health history; and any prior results you choose to attach to a request.
2. What they do with it
The Lab Providers use it to decide, in their own professional judgment, whether to authorize each panel; to register your order at a draw site; to collect and process your sample; to review results before release; to contact you directly if something needs prompt clinical attention; and to meet their own legal and professional obligations.
Inside their practices this information is protected health information under HIPAA, covered by their own privacy notices, which the App shows you in the lab ordering flow.
3. Telehealth Evaluation
Telehealth involves the use of electronic communications to enable healthcare providers and patients at different locations to communicate for the purpose of the patient receiving healthcare services. The telehealth visit that is available to you through the App is asynchronous meaning that you will submit your health information electronically and it will be reviewed by a Lab Provider at a later date. The Lab Provider will communicate with you via electronic means via the App.
The Lab Providers rely on the accuracy of what you tell them. The information you provide is accurate and complete to the best of your knowledge.
4. What are the expected benefits of a telehealth visit?
The expected benefits of telehealth services include improved access to laboratory testing while you remain at your local location (e.g., home or office).
5. What are the possible risks of a telehealth visit and laboratory testing?
The possible risks of telehealth services include delays in evaluation and treatment due to deficiencies or failures of the equipment and technologies. Occasionally, a Lab Provider may determine that the transmitted information is of inadequate quality, thus necessitating another telehealth consult; or in rare events, a lack of access to complete medical records may result in judgment errors.
A blood draw carries the usual minor risks, bruising, lightheadedness, occasionally fainting or infection. Samples occasionally yield incomplete or inaccurate values.
6. Will my telehealth visit be private?
We use telehealth technology that is designed to protect your privacy. If you use the Internet for the telehealth services, use a network that is private and secure. Even if you use a private and secure network, there is a very small chance that someone could use technology to “hear or see” your telehealth visit or that there may be a breach of privacy.
7. What if I need follow up care?
You are responsible for consulting with your own healthcare provider for any follow up care based on your laboratory results.
8. How much does a telehealth visit cost?
The cost of your telehealth visit is included in the cost of your lab testing.
9. You can opt out of this entirely
You can opt to not get any laboratory testing or to get laboratory testing through your own provider. Your membership is unaffected either way. Where your state requires extra telehealth disclosures, the App shows you that state's version.
10. How long it lasts, and withdrawing it
This consent covers lab orders during your membership until you withdraw it, at any time, in the App or by emailing members@octohealth.com with REVOKE CONSENT in the subject line. Any clear written request to withdraw will be given effect, whether or not it follows this format. Withdrawal is effective on receipt and is implemented within ten business days. It works going forward, does not undo what was already shared, and pauses lab ordering until a new consent is in place. You can request a copy of this Consent at any time.
11. Who can I contact if I have questions about this consent?
If you have questions about this consent, you can contact members@octohealth.com.
By agreeing to participate in the telehealth visit and obtain lab testing, you are agreeing to the following:
• I consent to receiving asynchronous telehealth services in connection with my request for laboratory testing. I understand that the Lab Provider will determine whether or not specific laboratory tests are right for me.
• I understand that there is no guarantee that I will be issued an order for lab testing and that the decision of whether lab testing is appropriate will be made in the professional judgment of the Lab Provider.
• I understand the benefits and risks of an asynchronous telehealth visit and lab testing.
• I understand that I may withdraw this consent at any time.
Questions about any of this go to legal@octohealth.com.
